To complete your registration, please make sure you download, print & sign the Player Agreement, Waiver and submit payment for the Player Fees. Uniforms can be purchased at a later time if required in person or online.

We will also need a copy of your child's birth certificate and proof of insurance. Once completed, you can either email copies of the aforementioned documents to info@sealsfootball.com or provide them to your coach in person.
